HIPAA Notice of Privacy Practices

Effective Date: May 30, 2026

This notice describes how medical information about a client may be used and disclosed and how a client may get access to this information. Please review it carefully.

SevynCare is committed to protecting protected health information ("PHI") as required by applicable law. This Notice describes how SevynCare may use and disclose a client's PHI, the client's rights regarding that information, and how to file a complaint.

What Protected Health Information Means

Protected health information includes individually identifiable information relating to a person's health condition, care, or payment for care that is created, received, maintained, or transmitted by SevynCare in connection with services.

How PHI May Be Used and Disclosed Without Additional Authorization

SevynCare may use and disclose PHI as permitted or required by law, including for the following purposes:

Treatment and Care Coordination

PHI may be used or disclosed to coordinate services, communicate with a physician, hospital, therapist, home health provider, family representative, or other care participants involved in the client's care, as permitted.

Payment

PHI may be used or disclosed to obtain payment, submit claims, verify benefits, process invoices, or support billing and collections.

Health Care Operations

PHI may be used or disclosed for quality improvement, staff training, auditing, licensing, accreditation, compliance, business management, and general operational activities.

Appointment and Service Communications

SevynCare may contact a client or authorized representative regarding scheduling, reminders, follow-up, staffing, or care-related updates.

Persons Involved in Care

PHI may be disclosed to a family member, personal representative, caregiver, or other person involved in the client's care or payment for care when the client agrees, when the client has given opportunity to object and does not object, or when otherwise permitted by law.

As Required by Law

PHI may be disclosed when required by federal, state, or local law.

Public Health and Safety

PHI may be disclosed for public health activities, abuse or neglect reporting, health oversight, serious threat prevention, workers' compensation, law enforcement purposes, judicial or administrative proceedings, coroners, medical examiners, funeral directors, organ donation, or other permitted governmental functions.

Uses and Disclosures Requiring Authorization

Except where otherwise allowed by law, SevynCare will obtain written authorization before using or disclosing PHI for purposes that require authorization. A client may revoke an authorization in writing at any time, except to the extent action has already been taken in reliance on it.

Client Rights

A client generally has the right to:

  • Request restrictions on certain uses or disclosures of PHI, though SevynCare is not always required to agree.
  • Request confidential communications by alternative means or at alternative locations.
  • Inspect and obtain a copy of certain PHI maintained in a designated record set, subject to limited exceptions.
  • Request an amendment to PHI believed to be incorrect or incomplete.
  • Receive an accounting of certain disclosures.
  • Obtain a paper copy of this Notice upon request.
  • Choose a personal representative to act on the client's behalf where authorized by law.

Duties of SevynCare

SevynCare is required to:

  • Maintain the privacy of PHI.
  • Provide this Notice of Privacy Practices.
  • Follow the terms of the current Notice.
  • Notify affected individuals when required following a breach of unsecured PHI.

Complaints

A client may file a complaint with SevynCare if the client believes privacy rights have been violated. A client may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. Retaliation will not occur for filing a complaint.

Changes to This Notice

SevynCare reserves the right to revise this Notice and make the revised Notice effective for PHI already maintained as well as PHI received in the future. The current Notice should be made available upon request and, if applicable, through the website.
